Contract of Enrolment

1.  I apply for a place at Carnegie International Camp 2008 in respect of the child indicated on this form. I make this application with the intention that I shall be bound to the agreement with Leeds Metropolitan University should this application be accepted and that I am making this application on behalf of the child indicated on this form so that he/she is also bound by the terms of this agreement. To the extent that this agreement should ever be found not to be binding on the child I agree that this shall have no effect on my obligations under this agreement.
2.  I agree that neither Leeds Metropolitan University nor any of its staff or the camp staff shall be liable or responsible for any losses of
   or damage to property belonging to me or to the child. I understand that it is advisable to ensure that such property is the subject of
   appropriate insurance and personal care.
3. I agree to hold Leeds Metropolitan University harmless and to indemnify it against any liability or loss it may incur or suffer by reason
   of any acts or omissions of the child against other participants in the Camp, staff members or others whatsoever or any property
   belonging to whomsoever.
4. I agree to hold Leeds Metropolitan University and its Camp staff harmless and to indemnify them against any claim seeking to
   impose liability beyond that anticipated by this application/agreement.
5. I agree on my behalf and on behalf of the child that neither Leeds Metropolitan University nor its staff shall have any other liability for
   any loss, injury or damage however arising except so far as such loss cannot be excluded by law.
6. I give permission for the child to take part in any activity or trip organised as part of the camp programme on or off the Camp
   Residential Site. In the event of an emergency, I give permission for suitable medical staff selected by the staff of Leeds Metropolitan
   University to admit the child to hospital, xrays,
   secure proper treatment for, or injection, anaesthesia or surgery for the child.
7. I understand and agree that data from the Carnegie International Camp 2008 may be used by Leeds Metropolitan University for the
   purpose of academic research. Any such research will use the data for statistical purposes only, and will not identify the individuals
   by name. I agree that Leeds Metropolitan University retains all copyright to Carnegie International Camp and reserves the right to
   use photographs, statements and other media from all campers in future publicity or academic research.
8. I agree, and agree on behalf of the child, to abide by the rules and regulations of the Camp and Leeds Metropolitan University.
   Conduct that, in the considered opinion of the appointed staff of Leeds Metropolitan University, is not in the best interests of the
   University or the conduct of good order at the Camp may result in the discharge of the child from the Camp without benefit or refund.
   No allowance or refund will be made in the event of late arrival or early departure
9. In agreeing to this contract you agree to the University processing personal data contained in the enrolment form about you/your child
   and other data which the University may obtain for any purpose connected with this contract, or for any other legitimate reason. The
   University will handle all such information in accordance with the DPA and with due regard to confidentiality.
10. I confirm that if this application is accepted, the balance due will be paid by June 1 st 2008. Failure to pay by June 1st 2008 may lead
    to the place on the Camp being lost. I confirm that deposit of £250 is non refundable after April 1 st 2008 in the case of cancellation,
    failure to pay the balance or as per No.8 of the Contract of Enrolment.
11. This agreement represents the entire understanding and contract between Leeds Metropolitan University and myself and the child
    and supersedes all and any representation or agreements which may have been made or taken to have been made by or on behalf
    of Leeds Metropolitan University other than those contained herein.
12. Leeds Metropolitan University reserves the right to cancel Carnegie International Camp, though we make every effort to ensure this does not occur. In the event of the Carnegie International Camp being cancelled by Leeds Metropolitan University, alternative dates will be proposed or a full refund of monies paid will be made to the customer. Leeds Metropolitan University takes no responsibility for loss of profit and/or for any incidental, consequential special or indirect losses as a consequence of exceptional cancellations.

Please enrol my child . I have read the Contract of Enrolment terms above, am familiar with them, and agree that this enrolment is acceptable to me and is subject to everything contained therein.
